Check the method of ticket-delivery you prefer.
NOTE: Remember to include a self-addressed stamped
envelope if you want us to send your tickets to you.

Select the quantity and the type of dinner ticket you're
buying.
NOTE: If you choose the Family option, enter the number
of family members.

Select the quantity of raffle tickets you're buying.
NOTE: On the <raffle
prize-package list>, enter the appropriate number
of tickets purchased beside your appropriate choice of
prize(s). Remember to include this preference form
with your order form.
6.
Add up the individual quantities and prices, then include the Total Due on your order form.
7.
If eligible, enter the <Big
Spender Drawing>.
8.
Give the completed order form, completed prize list, and
the total amount of money due, together, to Mrs. Hogan
or to Mr. Sherk at the Music Department
or
mail
the completed order form, the completed prize list, and
a check (or money order) for the total due, together, to
